Background Check Disclosure and Authorization

Disclosure (stand-alone)

MASTERDO INCORPORATED, operating the Zounya platform ("Zounya"), may obtain one or more consumer reports or investigative consumer reports about you for purposes of evaluating your eligibility to access and continue using the Platform as an independent Braider. These reports may include information about your identity, criminal history, sex-offender-registry status (where applicable and legally permitted), and other eligibility information obtained from a consumer reporting agency.

This disclosure is provided to you as a separate, stand-alone document. It contains no liability waiver and is not part of the Braider Independent Contractor Agreement.

Your rights under the FCRA

• You have the right to request a copy of any report obtained about you.

• You have the right to a summary of your rights under the federal Fair Credit Reporting Act (FCRA), which will be provided with this disclosure.

• If information in a report may lead to a decision that adversely affects your access to the Platform, Zounya will, where required by law, provide a pre-adverse action notice with a copy of the report and the summary of rights, allow a reasonable period to dispute the accuracy or completeness of the information, and provide a final adverse action notice if the decision is confirmed.

Authorization

By checking the box and clicking "I authorize," I authorize MASTERDO INCORPORATED (Zounya) and its third-party consumer reporting agencies and providers to obtain and use consumer reports and investigative consumer reports about me for eligibility, onboarding, and ongoing-eligibility purposes, on a recurring basis during my use of the Platform, to the extent permitted by applicable law. I understand my access to the Platform may be affected by the results.

Vos droits (FCRA) : demander une copie du rapport ; recevoir un résumé de vos droits ; en cas de décision défavorable, recevoir un avis préalable avec copie du rapport, disposer d'un délai raisonnable pour contester, puis recevoir un avis final le cas échéant.

Autorisation : en cochant la case et en cliquant sur « J'autorise », j'autorise MASTERDO INCORPORATED (Zounya) et ses prestataires à obtenir et utiliser des rapports de renseignements me concernant à des fins d'admissibilité, sur une base récurrente, dans la mesure permise par la loi.
